This pre-assessment tool can be used to obtain valuable information from primary communication partners about a communicator's current communication skills (receptively and expressively) and preferences.  This survey is particularly helpful in outlining the current communicative means and functions the communicator is expressing in familiar contexts, with familiar partners.  You can provide this survey prior to your AAC assessment as a means of gathering valuable data and as a time saver, cutting down on interview needs and helping to prioritize direct assessment activities during your real-time session.    See sample pages in the preview images (not the full survey).
